SKI TRAINING & SKI TRIP 2026 WAIVER

As we prepare for our upcoming ski resort trip, we are holding a mandatory dry-slope training session on 12th December to get everyone ready and confident on the snow!

Baba Yangu Foundation is committed to protecting all young people. Please complete the following information to ensure safety during participation in the SUSO Project. A parent or guardian should complete this form if the participant is under 18 years of age.

Note: if consent is not given, the participant will not be able to take part in the ski trip activity.

Skiing is an inherently risky and physically demanding exercise, which the participant and/or their parent(s) or guardian understands, acknowledges, and agrees.

While participating in activities during excursions with Baba Yangu Foundation, the participant and participants’ guardian acknowledges all responsibility for any risk of injury, property damage, or death.

Participant or guardian hereby waives any and all claims they may have against Baba Yangu Foundation, as well as their subsidiaries, affiliates, partners, officers, directors, employees, agents, and volunteers, arising out of Participant's engagement in the programmes activities, including skiing and ski training.

If over 18years of age, the participant confirms that the Activities are to be undertaken in accordance with the safety rules made available prior to participation and the advice given in the pre-participation safety briefing to be undertaken prior to participation, together with any oral instructions or advice given prior to or during the session.

For an underage participant, I confirm that I am responsible for the child/ren in my care and undertake to ensure that he/she/they undertake the Activities in accordance with the safety rules made available prior to participation and the advice that I and he/she/they will be given in the pre-participation safety briefing to be undertaken prior to participation, together with any oral instructions or advice given to me and he/she/them prior to or during the session.

Participants' use of any rental equipment, and the surrounding areas may be terminated without return if they do not respect these norms of conduct.

I understand that my child needs to follow the behaviour code and any safety rules so that Baba Yangu Foundation can keep them and other children safe.

I certify that to the best of my knowledge I (if over 18) the participant is/are in good physical condition and do not have any medical condition which might have the effect of making it more likely that he/she/they be involved in an incident which could result in injury to him/her/themselves or others.

Any photographs, videotapes, recordings, or other records taken while on a Baba Yangu Foundation excursion may be used for publicity by BYF.
